The San Francisco Giants may have just received their first real gut punch of the spring, and it comes at a time when pitching depth was already a concern.

According to NBC Sports’ Taylor Wirth, right-hander Hayden Birdsong is seeking a second opinion after an MRI revealed a Grade 2 forearm strain and a sprain of the ulnar collateral ligament (UCL), a combination that frequently puts pitchers at a crossroads between rehabilitation and surgery. The 24-year-old will travel to Dallas to meet with renowned orthopedic specialist Dr. Keith Meister, demonstrating how serious the situation has become.

This is more than just an injury update for a team that entered camp with internal pitching options behind Logan Webb and Robbie Ray; it is a structural concern.


Second Opinion Signals Uncertainty—and Risk

When a pitcher seeks a second opinion for a UCL-related injury, it usually means one thing: the initial diagnosis allows for a variety of outcomes, none of which are ideal.

A Grade 2 strain indicates partial tearing, and many pitchers in this situation eventually require Tommy John surgery. If that path is taken, Birdsong will almost certainly miss the entire 2026 season and possibly a large portion of 2027. Even in the best-case scenario, the rehabilitation process is likely to take months rather than weeks.

The Giants must strike a difficult balance as a result of this uncertainty. Do they expect Birdsong to return late in the season? Or will they view this as a lost year and aggressively seek reinforcements?

Manager Tony Vitello acknowledged the gravity of the situation, saying to NBC Sports that it was “not the best-case scenario,” but the underlying reality is even harsher. Birdsong represented the upside.

He showed promise early last season and went into spring training working on a new cutter to complement a fastball that has reached 99 mph. That kind of development is important for a Giants team looking to close the gap between its established arms and its next generation of contributors.

That bridge has now been compromised.

The Giants will most likely rely more heavily on younger players like Carson Whisenhunt and Trevor McDonald, while also reevaluating bullpen depth, which is already lacking in consistent swing-and-miss production. Non-roster arms may suddenly become critical pieces, rather than insurance policies.

There’s a broader implication here. Injuries like this do more than just shorten innings; they also alter development timelines.
